Payment Options
Payments can be made by cash, check, credit or debit card at the time of treatment. We also accept CareCredit and Cherry financing.
We accept most major dental insurances and are preferred providers with Blue Cross Blue Shield, Delta Dental, and several others.

Payment:
●Payment is due in full on the day of service.
●We accept cash, check, debit, and credit cards.
●Estimated insurance co-pays are due at the time of treatment.
●We do not offer in-office payment plans.
●Third-party financing options (Cherry and CareCredit) are available.
Appointment Confirmation:
●All appointments must be confirmed in advance.
●Unconfirmed appointments may be cancelled or reassigned.
Cancellations
●48-hour notice is required to cancel or reschedule.
●Less than 48 hours’ notice and No-shows = $50 missed appointment fee.
●Fee must be paid before scheduling future appointments.
●Repeated no shows may result in dismissal from the practice.
Late Arrivals:
●Please arrive 10–15 minutes early.
●Late arrivals may be rescheduled.
●Repeated lateness may result in dismissal from the practice.
New Patient Forms
We ask that all new patients complete a patient information form, a health history form, and agree to our financial policy. These forms are available below for your convenience.
Emergency & After Hours Care
Dental emergencies are never convenient or timely. We are committed to our patient's dental health and comfort. If you have a dental emergency after regular office hours, please call our office and leave us voicemail and we will get back to you as soon as possible.
Handicap Assistance
We have a handicap ramp located at the back entrance of our office building, so you do not have to climb steps. Please call our office at (615) 822-2602 if you need any assistance.
