Payment Options
You can pay for your office visits and any treatments by cash, check, credit or debit card at the time of treatment. We also accept CareCredit.
We accept most major dental insurances and are preferred providers with Blue Cross Blue Shield, Delta Dental and several others.

Dental Insurance: Estimated Co-payments (the part that is not covered by your insurance) are due on the day of service. You must provide a completed dental form with assignment of benefits to the doctor.
Payment Plans: Payment plans are not offered. We recommend applying for Care Credit if you would like to make monthly payments.
Children: The parent or guardian who brings the child into the office for treatment is financially responsible regardless of dental insurance or legal responsibility another parent or guardian may have to this child.
Missed Appointment: If you are unable to keep your appointment, we kindly ask for at least a 24 hour notice. We will not charge you for missing an appointment. However, missing more than one appointment without notice will result in dismissal.
Late Appointments: Please show up 10-15 minutes early to your appointment. If you’re late for your appointment, we reserve the right to cancel/reschedule your appointment. Constantly showing up late to your appointments will result in dismissal.

We ask that all new patients complete a patient information form, a health history form, and agree to our financial policy. These forms are available below for your convenience.
Emergency & After Hours Care
Dental emergencies are never convenient or timely. We are committed to our patient's dental health and comfort. If you have a dental emergency after regular office hours, please call/text (615) 289-8396 to reach our office mobile phone.
Handicap Assistance
We have a handicap ramp located at the back entrance of our office building, so you do not have to climb steps. Please call our office at (615) 822-2602 if you need any assistance.
